Carpathia Wellness Hub typically does not share your information with third parties unless they are involved in your care or you have consented to share data, such as instructing us to send your prescription to a pharmacy. CARPATHIA NATURAL HEALTH LIMITED FROM

1 THE SERVICES

1.1 These Terms and Conditions will govern any services or consumables provided by Carpathia Wellness Hub Limited to the Client, unless a separate written agreement, signed by both Carpathia and the Client, is in place. These Terms and Conditions take precedence over any other terms put forth by the Client or implied by custom or previous dealings.

 

1.2 By submitting a Pathology Request, requesting any other services outlined in the Services Guide, or placing an order for any Consumables listed in the Services Guide (collectively referred to as an ‘Order’), the Client is offering to purchase those Tests, services, or Consumables from Carpathia under these Terms and Conditions. Carpathia reserves the right to accept or decline any Order.

 

1.3 A contract (an ‘Agreement’) for the provision of Services and/or Consumables between Carpathia and the Client is established when Carpathia confirms acceptance of the Client’s Order in writing, logs the relevant Pathology Request in its information management system, or begins providing the Services, whichever occurs first. Any request for additional Tests (as described in the Services Guide) is considered a request for further Services under that Agreement, which Carpathia may choose to accept or decline. In case of any discrepancies between the Order and these Terms and Conditions, the Terms and Conditions will prevail.

 

1.4 Carpathia will provide the Services under the Agreement: 1.4.1 in accordance with recognized industry standards; 1.4.2 in line with the UKAS medical laboratory accreditation standard (ISO 15189); and 1.4.3 employing suitably skilled and experienced personnel.

 

1.5 While Carpathia will make reasonable efforts to meet the Test turnaround times specified in the Services Guide, it does not guarantee these times for any specific Sample.

 

1.6 The Services Guide specifies Sample rejection criteria. If a Sample meets these criteria, or if Carpathia deems the Sample unsuitable for Testing or is unable to perform the Testing, Carpathia may decline to conduct the Testing under the Agreement and may dispose of the Sample.

 

1.7 Carpathia may destroy or dispose of a Sample after Testing is completed or upon termination of the Agreement, unless otherwise agreed in writing with the Client.

 

1.8 In delivering the Services, Carpathia will adhere to all relevant laws regarding anti-bribery and anti-corruption, including the Corruption (Jersey) Law 2006. Carpathia will ensure that its staff do not engage in any activities that could be deemed an offence under the Corruption (Jersey) Law 2006.

 

1.9 Carpathia is dedicated to ethical trading and maintains a zero-tolerance policy towards modern slavery (including forced labor or human trafficking of any kind), human rights abuses, and child labour. In fulfilling its obligations under this Agreement, Carpathia will comply with all relevant laws and internal policies concerning anti-slavery and human trafficking.

 

1.10 Carpathia may utilise external laboratories to perform Tests.
